If your business is in your home, you must fill out the Home Occupation Tax Affidavit* and the Occupational Tax Form*. You may get these forms by clicking below. Click below and fill the form out on your computer, then print it out, initial and sign it, and bring it to the Planning Dept. on the second floor of City Hall. If your business is not in your home, you need to fill out the Occupational Tax Form by clicking below.
