  2. For each goal/objective, note the results in the column headed “Results.”
  3. Evaluate the overall results in the column headed “Evaluation of Overall Results” as follows:

  • Performance surpasses all expectations for position and significantly increases organization’s results
  • Activities often contribute to improved or innovative work practices.     

Exceeds expectations (4)

  • Performance surpasses all expectations for position and significantly increases organization’s results
  • Activities often contribute to improved or innovative work practices.

Meets expectations (3)

  • Exceeds criteria for successful job performance
  • Performance consistently meets expectations for position and sometimes exceeds them

Needs Improvement (2)

  • Incumbent may be on a learning curve to acquire new skills due to tenure or a change in responsibilities; OR
  • Incumbent successfully meets expectations in some areas, but requires significant improvement in areas critical for successful job performance

Unsatisfactory (1)

  • Performance is below established standards and expectations, damaging organizational effectiveness
  • Immediate improvement is required
